About Shareholders certificates..

As a shareholder, are you gonna send us any letter or certificate that shows we have this much money invested for our record?...how soon after we send the money to the investment account assuming thru PNB-Antique Branch.

thank you so much for your interest in investing for the medical center.  as soon as we can received your investment and upon filling the investor profile form (attached file), we will send you the confirmation letter and the certificate of long term promissory note upon full payment of investment... or if you have any representative here in antique, they can claim the certificate in your behalf.

attached are the 2 files (investor/donor profile form and brochure).
